This insurance provides an Accidental Injury Benefit of $5,000 per Injury and includes coverage for intercollegiate athletic sports Injuries.
Accidental Injury Insurance is provided for the student athletes of Livingstone College during the play and practice of Livingstone College sponsored and supervised intercollegiate athletic events. If an athletic injury occurs, the claim for medical treatment must be submitted by the athletic trainer.
All Livingstone College registered students are automatically covered under the Accidental Injury Insurance. The cost of this insurance is included in tuition and fees. This is not health insurance or a primary sickness or hospitalization plan; the insurance covers accidental injuries only (including those relating to intercollegiate athletics). Please see the Forms Section - labeled Certificates - for details. When the maximum benefit under the Student Accident program has been reached, additional benefits may be available to the student athlete under the Intercollegiate Athletic Insurance Plan (talk to your Athletic Trainer for more information about filing a claim for these benefits).

HOW DO I OBTAIN PRESCRIPTION MEDICATION
You may obtain your medication from any pharmacy, pay for your medication at the pharmacy, and then submit the
receipts for your medication, so that you may be reimbursed. Send these receipts, along with your completed claim
form to Student Assurance Services. Please see Filing Your Claim with Student Assurance Services.
For an accidental injury for which you need to claim benefits, you MUST send Student AssuranceServices a claim form, which you must complete in full. This information will provide a description of your accidental injury. This policy is an "excess" policy, which means that the claim must be submitted to any private insurance for payment first. Student Assurance Services will consider payment on medically necessary expenses not covered by private insurance, such as deductibles and co-insurance.
Medical expenses not covered by private insurance which are related to athletic injuries must be verified and submitted to Student Assurance Services by your athletic trainer. These expenses need to be itemized by your medical provider and procedure codes must be provided on their invoices.
Submit the claim form (labeled SASCLM-3 below) along with the Explanation of Benefits (EOB) paid or denied by any private insurance (along with medical bills) to the CLAIM OFFICE:
